The Council of Accountable physician Practices ("CAPP") is an association of leading medical groups nationwide devoted to fostering the evolution and development of the Accountable physician group model, which is a multi-specialty medical group model characterized by focus on quality, efficiency, innovation and a culture of measurement (hereinafter "Accountable Practices or the "ap model"). In an effort to address escalating health care costs, uneven quality of care and the difficulty of assessing efficiency and value, the ap model is designed to promote greater accountability for both the cost and quality of health care at the hospital and physician levels.
