Link entered into a plan of dissolution in a prior year. The plan was approved in july 2021, all assets were distributed in august 2021, and link was dissolved. Link's original purpose was to provide Medicaid Service Coordination (case management) to assist individuals with developmental disabilities in gaining access to necessary and appropriate services and supports within the home & community-based (hcbs) waiver services under the nys Medicaid program and to develop, implement, and maintain an individualized Service plan (isp) for each indvidual served.
