Catholic Health Care Transitions Services Inc

The organization is a non-profit Florida corporation established under the sponsorship of the archdiocese of Miami to operate a post- acute transition Services program by promoting safe Transitions from hospitals to home and skilled nursing, inpatient rehabilitation and assisted living facilities. Skilled nurses act as one on one coaches and are involved with a series of visits and phone calls to educate and empower the patient up to 30 days of hospital discharge reducing their chances of returning to the hospital.
