Atwater Area Living at Home BLOCK Nurse Program

The Atwater Area Living at Home BLOCK Nurse Program volunteers and professional staff provide many different services and support for area seniors including rides, chores, errands, light housekeeping, service coordination assistance including referrals to other agencies and connecting seniors to community support, caregiver support such as coaching and brief respite, delivery of meals to the senior's home, friendly visits in-home or by phone, personalized health-related self-care coaching, advocacy and guidance in how to effectively work with health care and various community support systems, structured health promotion, health education and community wellness programs (excercise, stress reduction) as well as socialization opportunities that provides connection to the community.
