Program Reporting & Evaluation:In 2025, the Foundation, its evaluation partners and its CHANGE grant awardees continued to refine their evaluation framework that centers community voices and measures improvements in Healthcare access. As part of the framework, awardees submit a mid-year and year-end impact report each grant year and receive an evaluation prepared by the evaluation partners. The evaluation helps awardees assess their progress toward goals and may include recommendations to address challenges as needed. Awardees have regular access to Foundation staff and evaluation partners to provide assistance. By late 2025, all 6 awardees met or exceeded their objectives overall on their evaluations and collectively reached 29,000+ patients through direct services and outreach. All 6 awardees successfully demonstrated improvements in Healthcare access such as through expanding service offerings and reach, addressing patient barriers and/or integrating patient-centered practices.
