Advocates for Healthy Citizens

Advocates for Healthy Citizens (dba healthnet gaston) is a collaborative care network dedicated to providing access to affordable health care for low-income, uninsured gaston county residents with chronic medical conditions. Enrollees are assigned a medical home with a primary care provider and are provided counseling and education on how to manage their chronic diseases and navigate the health care system. Health care services are provided at no cost to the patients by physicians who volunteer their time. Healthnet gaston also works with individuals experiencing homelessness to connect them with essential resources such as shelter, housing, food, and other supportive services.
