The Colorado Health Foundation

The Colorado Health foundation's ("the foundation") direct charitable activities support work to improve the Health of all coloradans. In 2024, this included the following: provided technical assistance to clinics throughout the state in building and improving their team-based care and financial practices with a Health equity lens. Facilitated a funders group focused on youth and their needs in Colorado. Provided technical assistance to community colleges in envisioning and implementing mental Health supports for students in their communities. Supported nonprofit leaders working on youth resiliency to share best practices and create a network to support these leaders' Health and well-being. Provided consulting evaluation support to survey grantees in the adult recovery portfolio to understand if they are billing Medicaid for peer support services and, if not, gauge their interest in further technical assistance to become a medicaid-approved provider. Provided technical assistance to primary care clinics to improve and expand their data collection and diversity, equity and inclusion practices.supported facilitation, coaching, leadership development and capacity building with community members and nonprofit organizations in nine communities across Colorado through the foundation's locally-focused work. Provided coaching, technical assistance, strategy development and implementation support with staff and community partners. Supported data collection, learning and insights that were then shared back with grassroots organizations. Compiled lessons, insights, learnings and recommendations for future use for civic engagement, such as census participation. Provided tracking and monitoring resources for local ballot measures and other policy actions. Conducted and disseminated a statewide survey that was made publicly available and widely distributed with advocates, policymakers, direct service providers, public officials and community members to help share the views and perspectives of coloradans. Hosted "at the heart of the matter," a virtual event series about Health in Colorado.continued to support grantee cohorts for the foundation's nonprofit sabbatical program, which supports long-term Colorado executive directors and their organizations through respite and organizational capacity building. Honored a community leader through the dr. virgilio licona community Health leadership award program. Commissioned and released polling research through bilingual campaigns to better understand coloradans' perspectives on their Health and quality of life through pulse: the Colorado Health foundation poll. Contracted with a communications firm to develop and oversee a social change communications strategy within our locally-focused work portfolio. Contracted with a language justice firm to support the foundation's bilingual engagement and strategy that includes ongoing translation, interpretation and transcreation needs for work across the organization.
