Better Health for Northeast New York Inc

BHNNY was created to participate in New York state's delivery system reform incentive payment program (dsrip) as the lead entity of a performing provider system serving albany, columbia, greene, saratoga, and warren counties and to implement the dsrip project plan submitted and approved by the New York state department of Health. The goals of the dsrip project plan were to create and sustain an integrated healthcare delivery system that could meet the needs of Medicaid and other eligible patients in an effective and efficient manner. BHNNY worked collaboratively with partner organizations to improve care, care access, and reduce avoidable and preventable utilization of the emergency department and in-patient hospitalizations, and ultimately reduce costs. Now that the nys dsrip program has concluded and all funds from the program were dispersed, BHNNY is inactive.
