5-2035 Global Foundation for Community Health

We are a Health policy formulation and implementation organization formed to address the dire situation in chronic noncommunicable diseases (preventable and related lifestyle conditions of type II Diabetes, coronary artery disease, hypertension, and stroke). Our model was principally developed at Harvard with participation and consultation with experts around the world over the last 13 years. The model is a frugally innovative Community-empowered Health policy implementation blueprint which we hope to make a Global model. The model enhances available healthcare infrastructure utilization and efficiency by recruiting Community members to participate actively in its Health and quality of life deployment and improvement. We will solicit funds from individuals and entities who share our 5-2035 vision. We will accept grant applications from domestic and international groups who set up a healthcare initiative based on the model. We will provide guidance to these recipient organizations.
