Services to residents of highbridge permanent Housing will be provided by samaritan daytop village (sdv) and will serve individuals and families with children who are homeless and who have a substance use disorder (sud) or serious mental illness (smi) and have a range of needs that require a variety of supportive services. To determine the best course of action for each client, sdv will conduct a comprehensive assessment to identify their specific needs and create a person-centered support service plan for each client that will be updated at least quarterly. Sdv's team of case managers, social workers, recovery coaches, benefits specialists, and employment specialists will perform thorough assessments to identify each tenant's needs. They will then address these needs through counseling, empathetic support, role modeling, and connecting tenants to community providers to help them maintain positive changes.
